Applying to Advanced Programs in CT and MRI

To ensure that your application to the radiography program at the UPMC School of Medical Imaging is complete, we are providing the forms in PDF format. Please print out, complete, sign, and date the following forms:

• application (pdf file)
• two letters of reference (pdf file)
• pregnancy policy (pdf file)
• "Dear Prospective Student" letter (pdf file)
• technical standards form (pdf file)
• application checklist (pdf file)

along with your:

• official high school and college transcripts, if applicable
• $25 nonrefundable application fee in the form of a check or money order payable to UPMC
